While each individual patient’s length of stay will be different, many short-term rehab patients are discharged within 30 days.
When your care team feels that you have gained maximal mobility and the ability to return home, the social worker and discharge planner will facilitate discharge. A multi-disciplinary Care Conference is held with family members to ensure that everyone is in agreement with the course of action recommended. The discharge planner will schedule any needed services, such as home health care and outpatient therapy, if necessary.
Many patients are referred to Aaron Manor prior to undergoing elective surgery. Aaron Manor maintains close working relationships with many orthopedic, cardiac and neurosurgery practices. Prior to surgery, patients are welcome to visit Aaron Manor to meet with an admissions representative for a tour and to reserve a room.
Other patients are referred to Aaron Manor from the hospital when it becomes apparent that they are not yet well enough to successfully return home.
A variety of conditions brings patients to Aaron Manor. Many come for post-surgical rehab, to ensure that they get the physical therapy they need to return home physically able to resume normal activities to the highest possible level.
Other patients are admitted from local hospitals with a variety of medically complex issues. The skilled nursing staff at Aaron Manor is well equipped to handle even the most seriously debilitated patients.
In addition to basic skilled nursing services, physical therapy, and occupational and speech therapy, Aaron Manor provides IV therapy, wound care, and other skilled services to meet every patient’s needs.
